Procedures for Registering with the Disability Services Center

The following guidelines were developed to assure that documentation will provide sufficient information to verify eligibility and to support request for accommodation in a timely manner.

Step 1:

Students must meet the admission requirements for New River Community College by completing the appropriate application for admission, providing transcripts from other schools as appropriate, and completion of placement testing. Alternate placement testing formatting and accommodations can be arranged in advance if requested by the student.

All NEW STUDENTS entering NRCC for the first time must register for a New Student Orientation session offered prior to the first day of class at www.nr.edu/welcome.

Step 2:

Students must self-identify to an academic advisor that he/she has a disability and would like to request assistance/accommodation with classes. Students with disabilities working with the Center for Disability Services need to present a written request for reasonable accommodations and the supporting documentation to the appropriate office. This request and information should be submitted as soon as possible upon completion of the college application.

Step 3:

Student must contact the CDS coordinator to schedule a meeting to discuss current policies and procedures and to complete the intake process with CDS staff as this is an interactive process.

Step 4:

Third party professional documentation of the condition requiring the need for academic accommodation is required and must be obtained by the student from a medical professional. The type of documentation needed varies depending upon the disability. IEP and 504 Plans are helpful but not sufficient documentation. Professional assessment and evaluation must be current (within the past three years) and needs to include specific diagnosis and clinical documentation of accommodation requested. Students must be evaluated at their own expense and provide this documentation to CDS staff for completion of an accommodation review.

Step 5:

An accommodation review of all documentation will be completed by the CDS coordinator within one week of receipt of all documents and the CDS coordinator. The CDS coordinator will notify the student once the accommodation review has been completed and the Faculty Accommodation Notification Forms (FANF) are available for pick-up by the student.

  • Student must pick up new FANF forms at beginning of each semester, provide one copy to each instructor for all face-to-face courses and have the instructor sign the signature page of the FANF to indicate that the student and the instructor have discussed the accommodations specific to each class. The student will return the signature page of the FANF with faculty signatures to the CDS staff.
  • It is the responsibility of the student to hand-deliver the FANF to all instructors. CDS staff will send a signed scanned copy of the FANF to online instructors only and copy the student with his/her NRCC email address.
